Below is the pre-approved criteria for COVID-19 Testing:
#1) Criteria for pre-approval of testing (select one or more if applicable):
Hospitalized patients (or patients in the ED with anticipated hospitalization) who have:
Fever AND signs/symptoms of lower respiratory disease AND no alternative explanation for symptoms (e.g., negative RVP)
Fever AND signs/symptoms of lower respiratory disease AND travel to a high incidence area under a CDC level 3 travel alert (Europe, China, South Korea, Iran, Malaysia) or who have been on a cruise within 14 days before symptom onset
Fever OR signs/symptoms of lower respiratory disease AND close contact with someone with confirmed COVID-19 within 14 days before symptom onset
Immunocompromised patients or those with chronic medical conditions that put them at higher risk for serious infection (diabetes, heart disease, chronic lung disease, chronic kidney disease) with fever AND signs/symptoms of respiratory disease
#2) Additional criteria for pre-approval of urgent in-house testing (select one or more if applicable):
Patients with respiratory distress who are admitted to the ICU (or anticipated ICU admission)
Patients who will undergo an inpatient aerosol-generating procedure
Patients who are immunocompromised or at high risk (as defined above) and admitted with respiratory distress
Patient meets criteria in categories #1 AND #2: Approved for in-house rapid testing
Patient meets criteria in category #1 alone: Approved for standard commercial testing
(Note: criteria in category #2 alone do not qualify for pre-approval of testing)
All Other Cases: ID approval will be required for COVID-19 testing
